Ellis Water Survey

Dear Ellis Water Customers,

You may have received a s survey in the mail from Ellis Water and 120Water.  These surveys are for the Lead Service Line Inventory project that the EPA is requiring from all Community Water Systems such as ours and are critical to compliance with the EPA’s rules.

Unfortunately, our partner 120Water, made an error while printing these surveys.  The error resulted in some customers receiving surveys to their address with the wrong customer’s name on them.  They have been made aware of the problem and are working to identify the cause and how to correct it.  We apologize for this error.

The return postcard and the QR code will both have a unique Location ID that is tied to the mailing address, not the name on the survey.  Please fill out the postcard and return it or use the QR code to complete the survey on your phone. 

It is possible that this survey will be resent with the proper name on them soon for any surveys that do not get a response.   You may also receive this notice more than once if we have your email address in both our billing system and you are signed up for alerts through elliswater.org.  We are trying to let everyone know what happened and that the survey can still be used.
